Long Term Conditions

COPD, ASTHMA, DIABETES, CORONARY HEART DISEASE, STROKE/TIA, HYPERTENSION, ATRIAL FIBRILLATION, HEART FAILURE, NON-DIABETIC HYPERGLYCAEMIA (PRE-DIABETES), CHRONIC KIDNEY DISEASE, MENTAL HEALTH PROBLEM, HAVE HAD BARIATRIC SURGERY

If you have one of the above conditions, you will be recalled annually in the month of your birth for a review.

If you have a respiratory condition only (asthma or COPD), you will be invited to book an appointment to see our Practice Nurse who will complete your review. This could be a face to face appointment or a telephone appointment.

If you have any of the other conditions listed, you will be invited to book a 30 minute appointment to see one of our Health Care Assistants. If you have your mobile number registered with us, you can book your appointment using a link that we will send to you to save you trying to call the practice. The Health Care Assistant will take your bloods, blood pressure and any other tests according to your condition. You may be required to submit a urine sample, so bring a sample with you to your appointment.

Your blood test results will then be sent to one of our Nurse Practitioners who will review them. If there is no action to be taken you will be recalled again the following year. If any action is needed, you will either be contacted by a member of staff with any actions (for example to repeat bloods) or our Nurse Practitioner will give you a call if she needs to discuss the results with you.

If you have a respiratory condition plus one of the other conditions listed, you will need two separate appointments. Please let this receptionists know when you call for your appointment.

Housebound Patients

If you are housebound, we will contact the District Nursing team in the month of your birth and arrange the necessary tests. Our Nurse Practitioner will then review your results and contact you if necessary, or we will arrange any repeat tests if needed. If you are housebound with a respiratory condition, we will ask our Practice Nurse to give you a call.
