How to Register

Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.

Standard Full Care Practice Area

We accept new patients for full care including all normal GP services who live within our inner area the practice boundary. Any of our existing patients who move house but remain within the outer boundary of the boundary map may remain registered with us if they wish. Anyone moving outside of the outer boundary will need to either re-register with another practice, or change their Cestria registration type to “Out of area patient”. The Out of Area Patient scheme was set up by central government in January 2015, and patients under the scheme get access to most normal care and services, but are not entitled to home visits by Cestria. Details of the “Out of area patient” scheme are given below. We apologise for the confusing nature of these arrangements, they are caused by a complex change in the national registration rules. 

Out of Area Patients

From January 2015 practices have been allowed to register patients who live outside the practice boundaries. Any such registrations are at  the discretion of the practice, taking into account the feasibility of being able to provide good GP services “at a distance”. This varies between patients and depends on medical and geographical considerations. 

Special registration requests and unusual circumstances patients may apply even if not covered by the guidelines detailed here. Such registration requests will be considered by a senior doctor and the practice manager on an case by case basis. 

It is vitally important that all parties are clear who will be responsible for home visits and emergency care. If an application to become an out of area patient is successful, it is on the understanding that the patient will not be able to receive home visits from Cestria, they must attend the surgery in person for all ordinary needs which cannot be sorted out by phone. If an “out of area” patient does need urgent attention close to home, or a home visit, this will be arranged via the NHS phone service 111. There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

Please note that if you were previously registered with a Doctor outside the UK then the Practice will not have access to your medical notes. All patients who register from abroad should contact their previous Doctor to obtain copies of their notes.

Named Accountable GP

You will be assigned to a doctor who will be your “Named Accountable GP” as required by the NHS, however, you can choose to be seen by any clinician as long as they are available.

You will be added to the practice list, not an individual doctor. You can ask to be seen by any of the doctors in the practice although it will not always be possible to see the doctor of your choice on a particular day and time.

Pre-register online

Step 1: Complete online registration form

Our online registration form will collect your details, a brief outline of your medical history and record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services.

Please complete one form for each family member.

Step 2: Upload scans of photo identity and proof of address

Please deliver to the practice copies of two forms of your identity.
One form of identity must contain a photo of yourself and the other must provide evidence of your current address. You may upload your forms online.

People of No Fixed Abode

If you are homeless or of no fixed abode but reside within the practice boundary, then you can still register with us.

Organ Donation

From 2020, adults in England will be considered potential donors unless they choose to opt out or are excluded. For more information please visit the Organ Donation NHS UK where you can also register your decision.
